Who We Are

Primrose Community Services is a Home Community-Based Services Waiver provider in the District of Columbia that provides support services to people with intellectual/developmental disabilities. We are able to provide these services across a variety of settings, including private residences, group homes and other supervised facilities (e.g., apartments), as well as community residential facilities.

Our MISSION is to provide support services to individuals with intellectual/developmental disabilities that promote self-determination, independence, increased access to community resources, and opportunities to be fully engaged in their communities.

At PRIMROSE, we aim to provide individuals with intellectual/developmental disabilities with quality care to support their goals, health, wellness, and to increase their independence and self-determination.

Services Delivery Philosophy


Primrose Community Services believes firmly in our person-centered philosophy. We are committed to supporting people by meeting them where they are and supporting them as they accomplish their goals. We do this by honoring their values, cultural backgrounds, strengths, and wishes while encouraging self-advocacy and promoting self-determination and independence.

List of Services

We have 27 years of experience across various settings (e.g., assisted living facilities, private residences, hospitals) supporting people with a myriad of diagnoses including developmental/intellectual disabilities, geriatric care, memory care, and psychiatric disorders. 

We provide the following services

 In-home support services are provided to people that reside in their private residences. Services are driven by the individual's ISP (Individual Support Plan) and are intended to provide training in support of everyday life skills and increased independence. We focus on providing assistance with developing the skills necessary to reduce/eliminate behavioral episodes by implementing a Behavioral Support Plan (BSP) and/or positive strategies. We additionally provide training on, and assistance in the monitoring of health, nutrition, and physical wellness. Finally, we aim to assist in performing personal care and household tasks that are specific to the needs of the person. We are able to provide additional services, based on patient need.
We provide non-medical assistance and supervision to support a person’s goals, desires, and needs as identified in their Individual Support Plan (ISP), and reflected in his or her Person-Centered Thinking and Discovery tools. Goals may be related to the person’s safety, promotion of independence, community integration, and/or retirement.
We are able to provide relief to a person’s family or primary caregiver in the event of scheduled or unscheduled time away from the person. This service can prevent gaps in the delivery of care services an individual may require. Respite care services may be provided in private residences or away from the home setting.
Supported living services are provided to individuals enrolled in the Waiver program who have limited informal support, and therefore have an assessed need for assistance with the acquisition, retention, or improvement in skills related to activities of daily living. These individuals require assistance with the development of social and adaptive skills that are necessary to enable the person to reside in the community and successfully participate in community activities based upon what is important them, as documented in his or her Individual Support Plan (ISP) and reflected in his or her Person-Centered Thinking and Discovery tools.
Host home services enable a person to retain or improve skills related to: health. activities of daily living (e.g., money management, community mobility, recreation, cooking, shopping, use of community resources, community safety), and to develop other adaptive skills needed to live in the community. 
We provide training, counseling, and other professional support services to uncompensated caregivers who provide care, companionship, and/or supervision to persons enrolled in the ID/DD Waiver. A family training services professional will conduct an assessment of family training needs within the first four (4) hours of service delivery, and develop a training plan with goals and techniques that will assist the ID/DD Waiver participant’s unpaid caregivers. The training plan will include measurable outcomes and a schedule of approved family training services to be provided, for submission by the Medicaid provider to DDS before services are delivered. 
Employment readiness services occur over a defined period of time with defined outcomes to be achieved over the relevant time frame. These services provide learning and work experiences (including volunteer work) where a person enrolled in the Waiver can develop general and specific strengths, along with skills that contribute to their hireability in a paid employment setting. A person receiving employment readiness services may pursue employment opportunities at any time to enter the general workforce.
